The client is a large-scale enterprise healthcare provider specializing in oral surgery and dental services.
The Case
The Client’s Request
Faced with limited visibility into the revenue cycle and manual workflows that no longer scaled, the client recognized that a custom multi-site healthcare RCM platform development was the only way to achieve enterprise-grade automation and deep analytics. However, the existing legacy Practice Management System, with no API, no SDK, and no direct database access permitted under the vendor’s EULA, significantly constrained the client’s ability to achieve digital transformation.
The engagement also required designing and building a cloud-native dental PMS from the ground up. It had to be a long-term replacement for the legacy system that would interoperate with the RCM platform from day one and eliminate the vendor dependency entirely over time.
The client needed a vendor with proven expertise in healthcare software development, specifically a strong track record in dental app delivery, deep knowledge of legacy system modernization, and experience with RCM implementation services. Velvetech met all those criteria and was engaged to lead the initiative.
Challenge
Our initial objective was to enable direct access to and synchronization of key data, such as claims, patient, insurance, and financial information, from the client’s core PMS. This would dramatically reduce manual bottlenecks and data latency.
However, without API, SDK, or database access, standard integration approaches were not an option. This left us with a fundamental challenge: how to deliver enterprise-grade automation and real-time visibility without modifying the core system or violating vendor constraints.
Process
Grounded in a deep understanding of the client’s pain points and vendor restrictions, Velvetech developed a solution spanning two integrated products: an RCM platform handling healthcare revenue cycle management, and a custom-built cloud-native dental PMS as a long-term replacement for the legacy vendor. We defined a conceptual architecture centered on a four-layer non-invasive data bridge, enabling enterprise-grade automation, analytics, and visibility within the closed dental healthcare ecosystem.
Layer 1: Non-Invasive Data Acquisition
With direct system access ruled out, the path forward required a non-standard engineering approach. While a direct DB read would have accelerated development, it carried the risk of vendor schema changes and license audits.
Instead, we designed a bidirectional data acquisition layer that combines RPA and structured report extraction, ensuring a controlled, auditable, and vendor-agnostic data flow from the PMS. The RPA flow is built on Microsoft Power Automate, selected for its bidirectional capability and cost efficiency at $180 per year, compared to alternatives like UiPath at $2,000–$4,000 annually. It supports operations like patient records, appointments, and claims, with every update running directly through the PMS interface, without touching the underlying database.

The report extraction pipeline, in turn, pulls structured financial and clinical data from the PMS’s built-in reporting module. This includes AR aging by category and provider, account balances, charges, and payments by period, giving the client full revenue cycle visibility.
In parallel, Velvetech developed a cloud-native dental PMS purpose-built to replace the legacy system on the client’s own timeline. It was designed from the ground up to integrate natively with the RCM platform, eliminating the report-extraction dependency as the client migrates, without billing continuity risk.
To keep the workflow seamless throughout the transition, we built a unified auth service that provides single sign-on across both systems.
Since the client would also use reporting data to train downstream AI models, we prioritized accuracy and consistency from the outset. Specifically, we employed an on-the-fly normalization engine, which significantly elevated the reliability of algorithmic decisions.

Layer 2: Pre-Submission Validation and Eligibility
The client’s next priority was eliminating data errors in claim processing at the point of entry. Our team introduced an intelligent validation layer that automatically checks patient and insurance records before a claim is submitted.
Currently, we are migrating to Stedi as a single clearinghouse for both medical and dental claim submissions, eligibility checks, and payment reconciliation. For payers not yet supported by Stedi, automated portal-based workflows bridge the gap. The system also validates CDT codes, account numbers, group IDs, and required documentation at the claim level.
Recognizing that “black box” automation is a liability in regulated environments, we designed this layer using a human-in-the-loop model. AI algorithms identify and flag potential issues, but no claim is submitted autonomously. All activity is recorded in a HIPAA-compliant audit trail.
Patient data is encrypted via Azure Key Vault, with PCI-DSS controls across the payment path and tenant isolation enforced at the data layer. Each clinic’s records, billing data, and portal access remain logically separated, even across shared infrastructure.
Layer 3: Real-Time RCM Intelligence
One of the biggest challenges the client faced was data latency. To address this issue, our team built an external Revenue Cycle Management intelligence layer that consolidates financial and operational data into a unified dashboard. It delivers real-time visibility into patient AR aging, insurance claim aging, and collections, updated every 4 hours rather than monthly.
We intentionally kept this layer external to avoid the constraints of the legacy PMS and ensure continuous up-to-the-minute data access and visibility. This decision carried a broader strategic implication: by building intelligence outside the legacy PMS, the client is no longer bound by that vendor’s roadmap. The architecture’s resilience was tested directly: the RCM layer is decoupled by design, so a future PMS change would not disrupt the intelligence layer.
Payments flow through WorldPay directly into the ledger, covering check-in/check-out, terminal, and online patient payments. This keeps revenue data accurate in real time without manual reconciliation. Overall, the RCM platform runs entirely on Azure, ensuring enterprise-grade security and scalability.

Layer 4: Referral Coordination
Without referral tracking in the PMS, the client couldn’t see where patients were coming from or whether referrals actually led to treatment.
To address this, we built a dedicated layer that captures referring-provider details against patient records and follows each case through the full treatment lifecycle. The system shows which practices send the most patients, how many convert to treatment, and the revenue each source brings in. This gives the client a clear view of ROI.
Referring providers get updates on patient outcomes directly through the platform, keeping everyone in the loop without leaving the system.
Outcome
Delivering RCM intelligence within a closed healthcare ecosystem, without touching the core system or violating a single vendor constraint, demands a level of architectural discipline that most integrations never require. It means prioritizing long-term reliability over short-term shortcuts at every decision point.
